Position Purpose:

Under physician supervision and with a considerable degree of autonomy, the Community Medicine Coordinator - Mid Level position provides direct and indirect support to Operations, Training Division, and Administrative staff as well as community member patients. This position will drive the creation and implementation of the District's Community Medicine program. This role tracks system utilizers; assesses patient needs; implements appropriate care plans for acute and chronic illnesses; and researches, catalogs, and refers patients to appropriate regional resources.
