Case Worker

A healthcare organization serving diverse patient populations is seeking a compassionate and detail-oriented Case Worker to support individuals and families in accessing critical social services and community resources. This role works closely with clients/patients to assess needs, coordinate services, and provide ongoing support to improve overall well-being.
Responsibilities:

* Provide social services support to clients/patients receiving or seeking public assistance, food stamps, medical assistance, institutional care, and/or protective services
* Conduct assessments to identify client and family needs
* Develop, implement, and monitor individualized social service plans
* Determine eligibility for social service programs and benefits
