Care Partner

As a Care Partner, you’ll guide customers through their care journey with compassion. Working closely with families, clinical teams, and external providers, you’ll develop holistic health plans, coordinate services, and ensure customers receive safe, person-centred care that enables them to age well at home or in a place of their choosing

This is a full-time role based out of either the Bega office or Moruya office . Travelling up to 2 hours for home visits
