In the Transition Care Program (TCP) at UnitingSA- We are about restoration and recovery- right when an older person leaves the hospital, helping them achieve their personal goals and regain their independence so they can live confidently at home. We’re on the lookout for a collaborative, person-centred, and organised Care Partner to join our tight-knit, fast-moving TCP team.

What You’ll Do

As a Care Partner within the Transition Care Program (TCP), you are responsible for managing client recovery plans, which span a period of 6 to 12 weeks. You will:

• Work Collaboratively - Co-design personalised, goal-based restorative programs directly with clients, their families, and clinical teams.
• Navigate the System - Connect clients to a wide web of clinical services, including Allied Health, nursing and broader community aged care services...
