Stuart Family Health Center is looking to add a Care Coordinator to the team who will support Transitional Care. As a Care Coordinator in this role, you will work collaboratively with multidisciplinary caregivers across the continuum of care to provide coordination of care and disease management longitudinally to patients with chronic condition(s).
