Kirklees Council is excited to announce an opening for a Care Coordinator within the Personalised Care Team. The role involves maintaining a one-to-one caseload and supporting Integrated Neighbourhood Meetings across Kirklees.
This is a 12-month fixed-term position requiring the candidate to be flexible in hours and travel across the area. The successful candidate will undergo an enhanced DBS check and demonstrate outcomes agreed with the funder.
