Cardiac Rehabilitation Enhanced Practitioner

Job summary
We have an exciting opportunity for an experienced cardiac clinician to join the Bristol Hospital Group cardiac rehabilitation team. We are a fully certified service which meets the needs of over 600 patients annually who are recovering from a recent cardiac event. We can demonstrate excellent outcomes for our patients. We are innovative and ambitious, and work to provide the best possible benefits to our patients including through digital healthcare, close links with community services, wellbeing programmes and research.

A cardiac rehab enhanced practitioner will be the patient's first contact after leaving hospital, they will support the patient in making the best possible recovery and equip them with what they need to remain healthy long-term. They will work alongside the patient and the MDT to address cardiovascular risk factors, provide education about heart disease, advice on symptom management, and exercise.
