**Description:**

The Breast Health Nurse Navigator in the Comprehensive Women's Health Center provides patient care coordination by serving as a liaison between the patient and the practitioners and services she may require. The Women’s Health Navigator acts as a patient advocate, provides patient and family education, and coordinates all aspects of care to assure the provision of seamless comprehensive services. The Women’s Health Navigator plays a critical role in the patient experience including community outreach, optimizing patient services and working collaboratively with the team to create a patient centered environment. This involves developing expertise in the operations and systems of the enterprise related to promoting and maintaining health and wellbeing in women.
