ASSESSMENT COORD (MA/MSW) - PM SHIFT

Responsibilities Intake Clinician/Coordinator - Evening Shift FULL TIME The candidate selected will be responsible for responding effectively to requests for information on a wide range of human services needs. The position is responsible for direct contact with potential clients, consumers, residents, referral sources, physicians, clinical treatment team members and case managers. The position in expected to make an accurate assessment of inquirers' problems and needs, identify resources to meet their needs, make the appropriate referrals and provide advocacy when required.
