**POSITION DESCRIPTION:**

Provides ambulatory clinical pharmacy services (including disease management and comprehensive medication review and Population Health Management under a collaborative practice agreement at multiple sites within the organization.

**ESSENTIAL JOB FUNCTIONS:**

Provides pharmaceutical care by participating in patient care with the medical staff during outpatient clinic visits. This includes designing, implementing and monitoring patient-specific pharmacotherapy; promoting safe, evidence-based and cost-effective use of drug therapy. Performs Population Health initiatives including reaching out to patients, based on pre-determined clinical criteria to enroll them in pharmacist services in order to meet provider-delivered care management goals.
