Addictions Case Manager

About the Role

We are seeking an enthusiastic and experienced Addictions Case Manager, who is either an allied health professional or registered nurse with experience in addiction or mental health settings.

Responsibilities

• Provide comprehensive assessment, care coordination, treatment planning and recovery-focused interventions for individuals experiencing substance use and co‑existing mental health concerns.
• Facilitate medicated withdrawal where appropriate and support clients through withdrawal and recovery.
• Collaborate with clients, whānau, and support networks to promote freedom from addiction, enhance wellbeing and empower them to live fulfilling lives.
• Coordinate with a multidisciplinary team of clinicians and social service providers to deliver high‑quality care.
