About the Role

ACC Clinical Support Coordinator at Wairau Hospital will support day‑to‑day administrative and coordination tasks for ACC to prepare patients for discharge and transition to community care. This is a fixed‑term, part‑time (12months, 40hours per fortnight) role based in Wairau, working with a Nelson coordinator and the Allied Health Team to connect with clinicians, service providers, and whānau.

Responsibilities

• Ensure accurate and timely entry of patient information into the system.
• Respond to queries from clinicians, patients, and service providers professionally and promptly.
• Build and maintain relationships with service providers, whānau, and healthcare professionals.
